00:01:03.840 Nick Shirley in particular discovered Somali daycares that didn't have any children, home hospices without any patients, and of course, quality learning centers without the letter N.
00:01:13.960 But the following footage, which was shot by The Daily Wire's Luke Rosiak and his team, is maybe the single most revealing video that I've seen on this topic.

00:11:16.320 highest concentration of Somalis in the United States at around 70,000 Somalis.
00:11:22.540 And despite that fact, Ohio also happens to be one of the most reliably Republican states

00:11:28.400 Trump won Ohio three elections in a row, the most recent by more than 10 points, which
00:11:32.700 is the largest margin the state has seen since Reagan in 1984.
00:11:36.240 For all the statewide elected offices, except for judges, are held by Republicans.
00:11:40.100 And yet, as Luke Rosiak found, the fraud in Ohio is actually more egregious than anything we saw in Minnesota or California.
00:11:50.080 It's the kind of reporting that will make you wonder genuinely why you pay taxes.
00:11:55.820 I mean, what's the point?
00:11:56.580 Our government, whether it's run by Democrats or Republicans, Gavin Newsom and Tim Walls or Mike DeWine,
00:12:01.360 is completely uninterested in fulfilling its most basic responsibilities to its citizens,
00:12:07.140 its actual American citizens. Instead, at a minimum, they're complicit in ripping you off.
00:12:15.100 At a minimum. They're looking the other way while it happens right out in the open,
00:12:19.920 if not facilitating it actively themselves. Now, the office building from that first clip
00:12:25.540 is owned by a company called Cordoba Real Estate, which just so happens to own a lot of office
00:12:31.220 buildings in Columbus. And inside these Cordoba office buildings, you'll find hundreds of
00:12:36.520 businesses that collectively charge taxpayers more than a quarter billion dollars in just six years,
00:12:43.060 from 2018 to 2024. By itself, of course, this does not imply that Cordoba Group or these businesses
00:12:49.800 are doing anything wrong. But when you look closer at the numbers, you start to get a better idea of
00:12:55.240 what's actually going on. This is from Luke's reporting. Watch. Of the five buildings in
00:13:00.560 Columbus with the most Medicaid companies, three are owned by the New Jersey-based Cordoba Real
00:13:05.680 Estate Group, LLC, registered by Israel Izzy Steinberg and Isaac Nassar, who lives in a $4.5
00:13:12.920 million mansion in New Jersey, and showed off his sports car and one of his two airplanes online.
00:13:19.420 Altogether, Cordoba Real Estate Group owns seven buildings in the area during the time of the
00:13:24.160 Medicaid payments released by the federal government. These seven buildings are filled
00:13:28.200 with nearly 300 medicaid firms almost all the tenants in these buildings are medicaid businesses
00:13:36.520 who provide low-skilled usually non-medical care for elderly or disabled people the front
00:13:41.480 doors are open but inside the seven massive complexes are as abandoned as a post-apocalyptic
00:13:47.240 horror scene. Smoke detectors chirp for new batteries. Office doors have signs suggesting
00:14:04.080 the owner is out to lunch, even though the mail has been piling up. It's as if months
00:14:08.380 ago, a supernatural phenomenon whisked all life away without warning. The United States
00:14:13.380 government is under the impression that all these buildings hold thriving
00:14:17.640 businesses. In all, Cordoba houses 288 businesses registered with Medicaid,
00:14:23.340 which charts taxpayers more than a quarter billion dollars between 2018 and
00:14:28.380 2024. Collectively, they made 5.5 million Medicaid claims using billing
00:14:34.380 codes that are essentially unverifiable because the service happens in the
00:14:38.040 privacy of a patient's home. Now, watching this, you have to ask, how do any of these businesses
00:14:45.940 survive a single audit? If you're away for lunch for so long that the mail begins piling up,
00:14:55.460 it's a pretty good sign that you shouldn't receive a dollar in Medicaid funding.
00:15:00.380 In other footage, Rosiak found that one of these companies didn't even have knobs on the front door,
00:15:04.540 so it was totally inaccessible. This is flagrant stuff. It's as obvious as it can possibly get.
00:15:13.800 And yet, as far as I can tell, nobody in the government has done anything about it.
00:15:19.420 J.D. Vance posted yesterday in response to this story that the anti-fraud task force was going
00:15:23.740 to move into Columbus to investigate Rosiac's findings. But the question remains, why was
00:15:29.000 absolutely nothing done until this point. I mean, all you got to do is walk around the
00:15:34.320 building for five seconds and take a look around. A single site visit would raise all of these
00:15:39.660 issues. Why didn't anybody notice? Or more to the point, if they did notice, why wasn't anything
00:15:47.280 done about it? Now, given that Ohio now spends a billion dollars a year on home health, you'd
00:15:54.000 think that this would be a higher priority, but until Rosiak walked around, nobody in the state
00:15:59.360 or federal government appeared to notice or care about what was going on. Watch.
00:16:05.620 No windows on the outside helps hide the fact that there's nobody inside. But when you go in,
00:16:10.800 it's really obvious. The place is deserted. It's downright creepy. There is no one in any of the
00:16:15.900 offices. They're dark. Curtains are drawn. Some of them have notes that are like, we'll be back
00:16:21.860 in a few minutes and you can tell it's been that way for months. Now, the situation gets
00:16:25.920 more extraordinary when you consider the fact that Columbus only has around 6,300 people aged
00:16:31.960 75 or older who are on Medicaid. And yet Medicaid is something like half the state's budget
00:16:38.840 and Medicaid spending has doubled in recent years. So, you know, people aren't getting
00:16:44.780 drastically older. Instead, Columbus has imported a lot of Somalis and all of a sudden the home 1.00
00:16:50.580 health care industry is booming. Now, of course, the vast majority of these home health care
00:16:56.100 services don't benefit the larger community at all. I mean, they're run by con men and criminals
00:17:01.480 in some cases. Rosiac details one business that's owned by people with repeated convictions for
00:17:07.280 fraud, violence, and theft. And the state of Ohio is aware of these convictions, but nevertheless
00:17:14.040 allows them to operate a million-dollar Medicaid-eligible business.
00:17:20.060 I mean, it's absolutely staggering.
00:17:23.580 In another case, Rosiak exposed a woman who reinvented her janitorial LLC as a health provider,
00:17:30.000 then billed Medicaid nearly $100,000 the first month.
00:17:35.260 He also came across an accountant who lost his license for stealing public funds,
00:17:39.200 then opened a $7 million home health care company
00:17:42.020 using the address of a convicted money-launderer's teenage son.
00:17:47.160 Again, it's all incredibly blatant.

00:17:55.480 is that we're not uncovering some kind of cleverly devised hidden scheme.

00:24:30.380 Omega underwent a routine audit in August 2025, which found that in some cases,
00:24:34.520 on the same day that Omega said it went to people's houses to take care of them,
00:24:39.800 hospitals also billed Medicaid because they were inpatients there. Omega refused to submit an
00:24:47.080 official response to these findings, which led the auditor to recommend that Omega refund
00:24:51.600 just $1,000 to cover the discrepancy. But the auditor and the county apparently concluded that
00:24:57.760 there was no reason to doubt the truthfulness of Omega's other claims. So again, even when these
00:25:02.900 companies fail audits, it's not a big deal. There's really no consequence to them. You might
00:25:11.120 think that any significant discrepancy would cause a lot of problems for these LLCs, but that's 1.00

00:27:26.640 Now, building owner Ron Nevins, he agreed to speak with us.
00:27:30.160 He says he doesn't want to lose tenants by talking publicly.
00:27:33.120 But this, to him, is an issue of right versus wrong.

00:28:06.980 All things considered, Maine is a relatively small player in this national fraud, simply because the population is relatively low.

00:28:20.140 One doctor billed the government $120 million in a single year, claiming to oversee 1,900 patients.
00:28:27.720 With almost 2,000 hospice agencies, L.A. County has more than 36 states combined,

00:28:37.420 18% of the whole country's home health care billing is coming out of Los Angeles County.
00:28:42.580 How is that possible? And take a look at this map, a cluster of 287 hospice providers in a two-mile
00:28:49.980 radius, some in strip malls, unmarked buildings, even a wrecking yard and vacant lot. All of it
00:28:55.980 is just paperwork. I could fill that out in Kazakhstan if I want and get a hospice license
00:29:01.560 waiting for me. Now you saw on that chart that New York has a relatively low number of hospice
00:29:06.560 agencies compared to Los Angeles, so you might be inclined to think that for one reason or another
00:29:11.540 Maybe New York doesn't have as much Medicaid fraud as you'd expect, but indeed it does.
00:29:16.760 The single biggest job in the entire state of New York is home care.
00:29:20.900 It accounts for 650,000 people.
00:29:24.480 New York has more than 170 home health aides per 1,000 seniors, compared to the U.S. average of 68.
00:29:31.640 As RFK Jr. pointed out, family members are getting paid for groceries and rides to the doctor.
00:29:37.820 And HHS has no way of verifying any of it.
00:29:41.220 watch. In the old days, Medicaid paid for medical procedures. So if you got a hernia operation,
00:29:48.280 we could figure that out at CMS. We could see you had a scar. It was done by a licensed doctor,
00:29:55.860 by a licensed nurse, and we could track to make sure it actually happened. The waivers allow
00:30:02.140 people, family members, who are taking care of an elderly parent, who get paid for balancing
00:30:11.020 the checkbook, for picking up the groceries, for driving somebody to a doctor's appointment.
00:30:16.780 Today, the biggest job in New York, 650,000 people, the biggest employment niche is home care.
00:30:25.080 These are family members who are getting paid to do things that they used to do as family members
00:30:30.440 for free. And this is rife with fraud because we have no way at CMS to determine whether they
00:30:40.620 actually perform that duty or not. We don't know whether you drove your grandmother to a doctor's
00:30:46.040 office. Now, it's one thing to call out this fraud and send a task force to investigate it,
00:30:52.360 but there are more immediate direct changes that HHS could implement right now. For example,
00:30:57.260 within HHS, there's something called the Office of Refugee Resettlement. And if you go on their
00:31:03.340 website, here's what you'll find. Quote, the Refugee Family Child Care Micro-Enterprise 0.62
00:31:08.260 Development Program, established in 2013, helps recently arrived refugees start and run family
00:31:14.120 child care businesses in their homes to help generate an income and achieve self-sufficiency.
00:31:19.900 Many refugees come to the United States with experience caring for small children.
00:31:24.020 They may also have their own young children, which makes it difficult for them to work outside of their homes.
00:31:28.120 The program helps these refugees to use their child care skills to earn an income.
00:31:33.360 Child care businesses run by refugees may also help generate jobs for other refugees in a community.
00:31:38.500 The program provides refugee participants with training and technical assistance in professional child care and micro enterprise development,
00:31:45.140 assistance navigating the child care business licensing process, direct financial assistance to prepare homes for child care operations.
00:31:53.280 so in other words the federal government is actively helping foreigners obtain licenses
00:31:59.480 to run these scams hhs will guide these people through the process of defrauding the federal 0.56
00:32:05.100 government they even sell it to these foreigners as a kind of jobs program saying it can generate 0.61

00:32:35.120 lasting impact. The government's current approach, raiding these businesses, simply is not effective.
00:32:42.260 And speaking of which, the recent FBI raids in Minnesota targeted daycares that should have been investigated a long time ago.
00:32:49.400 This is the latest action that's been taken by J.D. Vance's Fraud Task Force, which is a great idea, but it's also too little too late. Watch.
00:32:59.380 I want to focus on those nine daycares raided by the feds.
00:33:03.480 We have learned the state sent them $67 million in the last eight years.
00:33:10.300 That's your tax dollars spent on this child care assistance program known as CCAP.
00:33:16.220 We look back at the last eight years, but I want to focus on just the last two years in state data that we obtained.
00:33:22.180 These nine providers went from collecting about $8 million in 2023 to more than $16 million in 2025.

00:35:40.400 They are then seen walking away from the man who was left hunched over on the ground as bystanders walk past him.

00:35:51.600 Police arrested Ahmed Abduali, Osman, 29, of Bellevue. Osman was later charged with second-degree assault. The second suspect was not arrested at the time, but after reviewing the surveillance video of the attack, police are now looking for him. Seattle police booked Osman into jail on the night of the assault on April 19th. He was released before a bail hearing while police were still investigating the case.
