00:11:01.780 Those who lost their lives were the Ocean Gate CEO, Stockton Rush, Titanic expert Paul-Henri
00:11:08.480 Narjale, British businessman Hamish Harding, Pakistani businessman Shahzada Dawood, and his
00:11:15.380 19-year-old son, Suleiman Dawood.
00:11:17.560 On June 18, 2023, the explorers embarking on a deep-sea mission to explore the wreckage
00:11:24.320 of the Titanic, resting approximately 12,500 feet beneath the surface of the Atlantic Ocean.
00:11:30.580 At approximately 1047 a.m., the Titan communications and tracking team, monitoring from a ship on
00:11:37.320 the surface, heard a bang.
00:11:39.920 For days, the nation watched and prayed as it remained not entirely clear whether the
00:11:44.640 vessel had imploded or simply malfunctioned and lost contact.
00:11:49.260 On June 22, debris found on the ocean floor, confirming the loss of the sub and the lives
00:11:55.180 of all on board.
00:11:56.660 The Coast Guard determining the cause of the incident, quote,
00:11:59.060 was the loss of structural integrity of the Titan's carbon fiber hull.
00:12:03.060 This loss caused the sudden catastrophic implosion of the hull.
00:12:07.860 The word failure, appearing 99 times in the report, painting a devastating picture of what
00:12:13.280 led to the tragedy, a, quote, toxic workplace environment, critical engineering failures,
00:12:19.300 and a series of ignored safety warnings.
00:12:22.580 According to the report, Ocean Gate CEO Stockton Rush was negligent in the events leading up to
00:12:28.260 the disaster, noting that Rush's dual role, CEO of the company and master of the Titan,
00:12:34.560 carried a heightened legal responsibility to ensure the safety and seaworthiness of the vessel
00:12:39.460 and its occupants.
00:12:41.220 Had he survived, criminal charges may have followed.
00:12:44.580 The report also finding that before the fatal trip in 2023, Ocean Gate had continued using the
00:12:50.480 Titan sub even after, quote,
00:12:52.680 a series of incidents that compromised the integrity of the hull.

00:13:02.760 last year on 60 Minutes Australia, explaining the Titan's construction was a recipe for disaster

00:13:10.060 You have a particular individual, Stockton Rush, he was enamored of the sort of the glamour
00:13:17.900 and the exoticism, and he was attracted to it, but he came from the wrong industry.
00:13:23.000 He came from the air, you know, from aeronautics, and aeronautical engineering is fundamentally
00:13:27.680 different, and he chose the wrong materials on the wrong approach because of the carbon
00:13:31.840 fiber.
00:13:32.520 And there's another issue, which is mating dissimilar materials together at a high-pressure
00:13:36.660 mating surface.
00:13:37.700 When you put titanium up against carbon fiber, they have what's, they have a differential bulk
00:13:42.100 modulus.
00:13:42.660 I don't want to get too technical here, but it means they contract and expand at different
00:13:46.520 rates, which causes slippage and stress and strain at that, at that interface.

00:13:52.520 I'm talking about a lot of pressure, and you're talking about certain rules you can't
00:13:55.700 break, and those rules were broken.
00:13:58.560 Despite longstanding concerns about the safety of carbon fiber under deep sea pressure, a new
00:14:03.960 Netflix documentary, Titan, the Ocean Gate Submersible Disaster, explores why Rush remained
00:14:09.920 fixated on using it.
00:14:11.900 He believed it could revolutionize the industry by slashing costs and expanding access.
00:14:17.140 When I looked at the cost of operating a sub, it's the ship.
00:14:21.000 The two things that drive the cost of submersible operations, it's not the replenishing the oxygen
00:14:25.660 or the carbon dioxide scrubber, that's nothing.
00:14:27.920 It's all about the ship, and the next most important thing is mobilization.

00:14:59.460 One example of that breakdown relayed by former Ocean Gate Director of Engineering, Tony
00:15:04.620 Nissen, who worked for the company from 2016 until he was fired in 2019.

00:15:59.140 Yeah, I told Stockton and said that there's probably a good chance that that hull is compromised, for sure.

00:16:32.460 able to operate Titan completely outside of the established deep-sea protocols.
