00:00:37.320 expected. The first thing that we want to do is identify what is exactly wrong with our nose right

00:03:09.500 true for gandy um but what is going to be true for him is this infraglobellar notch being a bit

00:04:37.680 um and you know that's very uh prevalent in gandy's side profile okay so let's go over the first
00:04:44.160 mechanism for soft maxing right so um the first mechanism that's going to give us probably the
00:04:50.360 biggest results uh you know for that one case that we showed at the beginning where you've got all
00:04:55.160 those blackheads all those sebaceous filaments that's causing your nose to be enlarged maybe causing a 1.00
00:04:59.880 little bit of extra publicity is going to be accutane right so accutane is overall going to 0.81

00:05:24.300 while accutane is going to down regulate the amount of sebum and over time we will get this
00:05:29.000 we could do one round of you know sebaceous filament extractions uh to give us that immediate thing
00:05:35.480 but the issue is a lot of the extractions uh you know the sebum is going to start to reaccumulate
00:05:41.260 again unless you're on accutane which is slowing down the production of sebum now the last thing
00:05:46.400 that i'll note is dutasteride dutasteride is an amazing other mechanism to down regulate the you know
00:05:52.920 production of sebum and down regulate your sebaceous glands from being overly active because
00:05:57.980 dht is a hormone that's responsible for up regulating the amount of sebum that your body
00:06:03.040 produces that's why a lot of people on steroids have that overly oily look it's not the testosterone

00:06:13.620 through the 5-alpha reductase enzyme we're not going to have dht uh anymore which means that you
00:06:20.020 know that up regulation of sebum will cease to exist so dutasteride is another amazing mechanism for
00:06:26.840 you know managing your acne and reducing dht now the second mechanism that we're going to go over
00:06:32.660 is use of topical over-the-counter hydrocortisone right we're not going to have to go with that
00:06:36.540 prescription grade super strong hydrocortisone because that could cause us some redness like
00:06:41.440 we see here um you know so what you're going to want to do is stick with that over-the-counter stuff
00:06:45.860 and use it every other day every other week now what that means is you know you use the topical
00:06:50.880 hydrocortisone on your nose to reduce the inflammation uh every other day but you know
00:06:56.340 we we do that for one week then we take a week off uh the reason for that being is obviously we
00:07:01.660 don't want to get the side effects of using corticosteroids uh so we're going to keep our
00:07:06.540 exposure super minimal and you're going to be able to maintain that you know decrease in
00:07:12.000 inflammation with this protocol in place right because that's not going to give you a permanent
00:07:16.620 change to use and implement the topical you know corticosteroids but it can be easily maintained
00:07:23.180 without overexposure to the drug now the second topical that i'm going to go over is topical
00:07:27.980 tretinoin now we already mentioned accutane but people are for some reason sometimes super against
00:07:33.440 the systemic version uh even though it's near the same mechanism uh so they'll cope with the topical

00:09:57.140 a super uh downturned tip this is going to alleviate some of that strain because that muscle
00:10:03.000 right here uh the depressor septinase is what's actually causing it to be downturned in a lot of
00:10:08.200 cases so if we paralyze it the nose could be a little bit more upturned a little bit more optimal
00:10:13.300 now i didn't do this because for my case my nasal tip is already slightly upturned but if you're
00:10:18.140 someone who's got like a nose that's like this uh you know we could alleviate some of the the pull
00:10:23.520 downwards and have a slightly more optimal nose through that regard and i'm someone who really
00:10:28.700 likes botox as a mechanism to protect against you know some of the facial muscle hypertrophy from
00:10:34.180 steroids obviously it's not going to really matter that much if you're using relatively moderate amounts

00:10:47.140 their facial muscles so botox could be a good way to protect you know your dimorphism a little bit

00:10:58.100 a lot of people even realize there's actually a small fat pad inside of the tip of your nose

00:13:26.960 knife sure this is going to be the next best thing and i would absolutely advise that but i don't really
